There are a variety of administration forms that have been included here for downloading. The completed forms should be forwarded to the Unit Administrator at the Institute along with any supporting documentation.
Personal Details Variation Form
This form is used when changing any of your personal details such as name, contact details, specific needs indicator or emergency contact. It is the responsibility of students to inform the Institute in writing of a change of address within seven (7) days of such change. If this written advice is not received, the Institute will not accept responsibility for Institute correspondence that does not reach the recipient.
Indication of Support Needs Form
If you have specific needs which may affect your ability to successfully undertake a unit of study, you need to inform the Institute, preferably before commencement or as soon as possible, to discuss your learning needs/support.
Application for Extension Form PDF or Word (doc)
If students find they are unable to submit an assignment by the due date, they must contact the Unit Coordinator and provide this completed form, along with any supporting documentation, before the due date.
Assignment Cover Sheet PDF or Word (doc)
This must be completed and included with each assignment. The PDF version can be downloaded and printed for handwritten completion. The Word version can be downloaded and completed electronically (or printed and completed by hand) for inclusion with emailed assignments. All assignments must include a cover sheet.
Enrolment Variation Form
This form is used to notify the Institute of your intention to withdraw or defer from a unit or course of study, or to request a leave of absence. Please see the "Student Fee Policy" for more information.
Application for Exemption from Workshop Form
This form is used to request an exemption from attendance at a workshop being held as part of a unit of study (NB: not all units have workshops). Complete the form and return prior to the start of the workshop. Please included the reason for the exemption request and attach any documentary evidence (ie medical certificate). Students will be advised in writing following a decision by the Unit Coordinator.
The Institute values feedback and encourages students to submit feedback forms evaluating aspects of course delivery. Feedback from students is incorporated into curriculum reviews and informs future course development and revision.
Any specific issues encountered by students while undertaking a course at the Institute can be directed to the Unit Coordinator, or to the Programme Director, Manager, or Administrative Staff.
This form should be completed and returned by the last day of semester.
